Here is the Partner Migration Checklist from the Australian Embassy of the USA:

INSTRUCTIONS FOR PARTNER (SUBCLASS 100/309) & PROSPECTIVE MARRIAGE (SUBCLASS 300) APPLICATIONS

Please provide the following items in order to apply for migration to Australia. It is strongly recommended that you submit your application using a method of trackable mail so that you can confirm the arrival of your package. We encourage you to submit as many of the documents as possible at the time of application, as this will significantly reduce the processing time. Please note, however, that medical forms will be sent to you upon receipt of your application if not included at the time of lodgment.

Processing fee; US $1075 (as of 1 July 2006). Cashier cheques or money orders should be made out to the Australian Government or complete the attached credit card authorisation. Personal cheques are not accepted.

Form 47SP (Application for Migration to Australia). Please ensure the entire form, as applicable, is completed to the best of your ability. Note that we are unable to help you complete the form.

Form 40SP (Sponsorship for Migration to Australia)

Three (3) passport type photographs of applicant and dependents who are migrating. All photographs must bear the names of the persons they represent.

Passports or other travel documents, valid for travel to Australia, for each person migrating. These should be signed. If you intend to use your passport during the 4-6 months of processing time, then please hold on to the passport/s until requested by the case officer and provide instead a notarised copy of the passport with the application)

Original (+ one photocopy) or notarised copy of marriage certificate(s). Only certificates issued by a government authority are acceptable. Do not send religious certificates. These must be provided for all previous marriages, as well as current, for both applicant and sponsor.

Originals (+ one photocopy) or notarised copies of birth certificates for all members of the family migrating to Australia. These must include parents' names. Only certificates issued by a government authority are acceptable. Do not send hospital or religious certificates.

Original (+ one photocopy) or notarised copy of sponsor's birth certificate. Only certificates issued by a government authority are acceptable.

Original (+ one photocopy) or notarised copy of sponsor’s Australian passport as well as evidence of status in the US, ie green card, visa (if applicable).

Original (+ one photocopy) or notarised copy of evidence of sponsor's legal status in Australia, ie evidence of Australian citizenship or permanent residence.

A detailed statement about your relationship with the sponsor from your first meeting to the present time, including important dates and events in the development of your relationship.

Police check for all applicants aged 16 and over from all countries where the applicant has resided in the past 10 years (including Australia). Instructions can be found on form 47P. Please note that all applicants currently living in the United States must obtain both:

- A police report from the state you are living in, AND
An FBI criminal history check – refer to the form 47P for details.

Original (+ one photocopy) or notarised copy of any legal name change for applicants or sponsor.

Original (+ one photocopy) or notarised copy of final divorce papers concerning all previous marriage(s) of either applicant or sponsor

Notarised declarations from family and friends attesting to the nature of your relationship. If completed in Australia, declarations should be made on form 888 (statutory declaration) – available from www.immi.gov.au.

Notarised statement from your sponsor detailing how they will meet the sponsorship obligations: ie, plans for accommodation and financial support.

Evidence of any cohabitation - eg, original or notarised copies of leases, utility bills, mail addressed to you both, joint bank accounts

Evidence of communication with your sponsor during any periods of separation - eg, letters, cards and envelopes, telephone bills, emails etc.

Medical Forms:

We strongly recommend that you do not complete medical forms until AFTER you have lodged your application. Completed medical forms must be sent directly from a panel doctor and therefore must be properly annotated with your file number. Medical forms will be sent immediately along with the letter confirming receipt of your application.

IF YOU ARE APPLYING UNDER THE PROSPECTIVE SPOUSE CATEGORY INCLUDE ALL OF THE ABOVE AND:
A LETTER FROM A MARRIAGE CELEBRANT IN AUSTRALIA SPECIFYING THE EXACT DATE OF MARRIAGE AND, IN MOST CASES, THAT A NOTICE OF INTENDED MARRIAGE HAS BEEN LODGED
EVIDENCE THAT YOU HAVE MET IN PERSON (IE, PHOTOGRAPHS TAKEN TOGETHER, ETC)

Please do not send documents relating to your educational qualifications or taxation forms. Additionally, please do not staple or clip documents, place them in plastic sleeves/binders or send bulky photo albums. It will speed the processing of an application if documents are left loose.

Oh and it looks like the fees have gone up as of Jan as I suspected they might.  It's not corrected on the checklist document yet, but it shows up here:

- the fee is now $1340 (for the prospective marriage visa subclass 300 lodged outside of australia)
- also i believe the applicant needs to submit 4 passport photographs now instead of 3

Just making sure:

The $1340 is PER family member included on Form 47SP, yeah? So I'm to send a $1340 money order for myself, and a $1340 money order for Ivy with the application. Or is the $1340 just for the ONE application? I mean, on the checklist it reads "processing fee", and there's only one application to process. I know I'm mincing words, but I'm getting all mixed up again since fees and forms seemed to have changed since I first had them downloaded a year or so ago! Blah!

Thanks for your help!

I'm so sorry, Christina... I bumped this thread to be helpful but I see it's confused you even further. Go ahead and keep doing what you're doing, and pay no attention to this checklist... it's out of date anyway. :(

Make sure you have the most recent forms available, they change them sometimes 2-3 times a year, at any old time, with no rhyme or reason.
When I was applying, I printed out everything, and 2 months later when I was ready to lodge it, the forms had changed and I had to print a new application, so make sure you have the most recent forms when you're ready to send them in! Also, prices change at odd times, so check first, before you send.

Lisa - thanks for the heads up. I thought about that today, as well, and checked my forms against what is currently available online at the immi.gov.au website. I remember reading somewhere that they change/update forms and the like in January, April, July, and October!

January, April, July, and October!

or May, or November, or anytime they feel like it, it's totally erratic.

LOL I'll definitely keep that in mind when I'm ready to lodge my application. I'll compare the website forms with that I've currently got and make sure they match. :)

By the way, I just sent my prints to the processing center in West Virginia. I requested 04/19 as my deadline. I hope they take that into consideration when processing them!
